Relay for Life permission slip

Riverbend is entering a team in the 2025 Penguin Relay for Life event to be held at the Penguin Athletic track beginning 1.00pm Saturday March 22nd and concluding 9.00am Sunday March 23rd. It is an overnight event.

For participants under the age of 18.

I agree for my child to be a participant in the Riverbend Relay for Life team under the supervison of Riverbend approved leaders. If they are not staying for the entire event, I agree to let the Riverbend organisers know when they are being picked up and/or dropped off. I also agree that they will follow the Relay for Life event guidelines and rules as well as the Riverbend Youth Code of conduct.

For participants over the age of 18.

I agree to follow the rules and guidelines of the Relay for Life organisers and, as a member of the Riverbend team, I will also abide by the Riverbend Youth Code of conduct and their Leaders code of conduct
